NON-PARTICIPATION CLASS REPORT AND OBSERVATION SHEET

 

Directions:  During the class period that you were not dressed and/or did not participate, you must answer the following questions by observing the activities that occur during your PE class.  You will need to use the back of this sheet to record your answers.

 

1.     The reason I did not get dressed for PE or participate in PE today is:

3.     Describe what activities and information is being learned in today’s class.

 

4.     Does the skill, activity or game seem easy to learn?  Explain in your own words why or why not.

 

5.     Write a short paragraph about what you know about this skill, activity or game.  Tell about strategies and/or skills involved in the activities.  If you have participated in this event before, tell whether you liked or disliked the activity.

 

6.     What are some things you can do next time so that you can dress and participate in PE class?

 

7.     Pick one person to watch in class.  Do not write his/her name.  Describe what he/she was doing throughout the class.  Be specific.

Dear Parent/Guardian,

         I regret to inform you that your child did not dress for his/her PE class today.  Consequently, it affects your child’s grade in PE class.  Should this practice continue, it could result in a failing grade.

 

Mattawoman Policy is as follows:

 

For each unit:  First Offense           Warning to student from teacher and written assignment

                  Second Offense        Letter/call to parent plus additional assignment

                  Third Offense          Referral to administration:  warning

                  Fourth Offense         Referral to administration:  possible suspension

 

This is your child’s __________ non-dress offense in this unit of ______________________________.  This form MUST be signed and returned.  If your child has a valid reason for not dressing, please contact me at 301-645-7708 or 301-753-1789 to discuss this issue.  You may reach me during my planning period from 1:45-3:00 pm.  Thank you for your attention to this matter.
