The Maryland Department of Health and Mental Hygiene Center for Immunization has released vaccination requirements for the 2024-2025 school year. All Charles County Public Schools students are required to have certain vaccinations prior to attending school.;
According to COMAR 10.06.04.06, a student who does not meet the immunization requirements may be temporarily admitted to or retained in school if the parent/guardian provides evidence of the student’s appointment with a healthcare provider or local health department to 1) receive the required immunization, 2) acquire evidence of age-appropriate immunity on the MD 896 form. The actual appointment should be no later than 20 calendar days following the date the student was temporarily admitted or retained.
Consumer Access to ImmuNet through Maryland MyIR Maryland residents can download/print out their official immunization records (such as the Maryland 896 form) without having to go to their healthcare provider or local health department through Maryland MyIR Maryland Sign in - MyIR Mobile (My Immunization Record), a free consumer access portal.
